An epic job title was listed for Ozzy Osbourne on his death certificate obtained by 'The New York Times' and 'The Sun' on Tuesday, Aug. 5. The former Black Sabbath frontman died at 76 on July 22.

Ozzy Osbourne's official cause of death has been revealed as a heart attack, according to his death certificate. Coronary artery disease and Parkinson’s disease were listed as "joint causes.” Osbourne died on July 22 at the age of 76 just weeks after reuniting with his former Black Sabbath bandmates.

Osbourne had been living with long-term health issues for years before he died on July 22 at age 76. Per the Times, Osbourne’s certificate officially said that the icon died of cardiac arrest, acute myocardial infarction, coronary artery disease and Parkinson’s disease with autonomic dysfunction.
